Position Details$7,500 Sign-on bonus Surrey PlaceDescriptionAssumes responsibility of Team Leader for the Interdisciplinary Care Plan Team to include in meeting Federal and State Regulations related to Resident/Client Care Plans and MDS.
Performs duties and responsibilities in a manner consistent with our mission and values.
Principal Accountabilities:

Serves as a liaison to other departments, medical staff, residents/clients and families to facilitate information, sharing and problem solving.

Maintains current MDS status of assigned residents/clients according to State and Federal guidelines.

Maintains most recent 15 months of MDS assessments for residents/clients residing in a certified bed.

Maintains and reconciles RUG tracking book of all Medicare/Managed Care clients for billing purposes and documents RUG codes and accurate billing information.

Collaborates with the billing department to resolve failed claims with Medicare and insurance companies.

Generates and distributes and updates weekly MDS and Care Plan schedule to members of the interdisciplinary team.

Determines need for and sets Assessment Reference Dates for unscheduled OMRA assessments (SOT/COT/EOT).
Identifies changes in clients/residents, communicates change and sets ARD for Significant Change in Status MDS as required by State and Federal regulations.

Prepares, facilitates and performs follow up of issues related to weekly O.

C Interdisciplinary Team meeting to review progress and care issues of assigned Clients/residents.

Trains, supports, monitors and oversees the duties of MDS support personnel.

Maintains appropriate computerized MDS programs.

Interviews client/resident, staff and authorized resident representative as needed and performs review of client/resident medical record to accurately complete MDS.

Completes accurate and timely data entry of coding into the MDS software.

Monitors completion of MDS data by other disciplines within timeframes prescribed by regulatory guidelines and advises supervisor of incomplete and/or untimely assessment by disciplines other than nursing.

Coordinates and provides educational in-services for nursing staff and other clinical Area staff related to ongoing Medicare part A changes as it pertains to the MDS and regulatory compliance.

ExperienceMust be a graduate of an accredited School of Nursing and currently licensed in the State of Missouri as an R.

or in the State of Illinois if applicable.
Should have a thorough knowledge of nursing principles and procedures, knowledge of MDS 3.
0, Federal Regulations, prior skilled nursing facility experience and basic knowledge of computer skills is required.
Assessment, care and treatment provided by the MDS Coordinator will be consistent with the specific age related needs of the patient.
The MDS Coordinator is competent to care for patient age groups including adult and geriatric patients.

Members of a comprehensive healthcare team, including other healthcare providers, the client/resident and the client/residents family and significant others.

Director of Nursing, Assistant Director of Nursing Supervises:
Nursing personnel, Data entry Unit Aide.
Working ConditionsEmployee is required to sit continuously and walk occasionally.
Employee must lift/carry a light (1-20 lbs) to medium (21-50 lbs) occasionally.
Employee may be required to lift over 100 lbs with assistance.
Employee may be required to bend, squat, kneel, climb, lift waist to overhead and reach.
For repetitive action, employee is required to perform simple grasping and pushing/pulling.
Employee may occasionally be exposed to the following environmental conditions:
electrical hazards, and CRT terminal work.
Employee may be exposed to blood-borne/air borne pathogens.
The most significant duties are included but this does not exclude occasional work assignments not mentioned or developmental duties.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
